At the 2019 FINA World Championships, a total of 3095 athletes engaged in competitions across the disciplines of swimming, diving, high diving, synchronized swimming, water polo, and open-water swimming. The success of head and neck cancer (HNC) treatment and subsequent prognosis depends heavily on early detection, diagnosis, and treatment; hence, a non-invasive, simple, reliable, and economical tool is needed for the same. The recent interest in salivary lactate dehydrogenase is directly related to the requirement cited above.
We seek to evaluate salivary lactate dehydrogenase levels in oral potentially malignant disorders (OPMD), head and neck cancer (HNC) patients, and a healthy control group, analyzing correlations between the parameters and determining potential gender and grade-based differences, to ultimately assess its use as a biomarker in OPMD and HNC.
To incorporate studies evaluating salivary lactate dehydrogenase in OPMD and HNC patients, a thorough search was conducted across 14 specialized databases and four institutional repositories, including those comparing or not comparing results to healthy control groups, as part of the systematic review. Employing a random-effects model and STATA version 16, 2019 software, a meta-analysis was performed on the eligible study data, utilizing a 95% confidence interval (CI) and a significance level of p < 0.05.
Salivary lactate dehydrogenase was the subject of evaluation across twenty-eight studies, encompassing case-control, interventional, and uncontrolled non-randomized designs. 2074 subjects, categorized as HNC, OPMD, and CG, were included in the study. A comparison of salivary lactate dehydrogenase levels revealed significantly higher values in head and neck cancer (HNC) when contrasted with both controls (CG) and oral leukoplakia (OL) (p=0.000). Likewise, oral leukoplakia (OL) and oral submucous fibrosis (OSMF) demonstrated significantly elevated levels compared to CG (p=0.000). Cases presenting with elevated SaLDH levels lend themselves to frequent follow-up and diagnostic procedures, such as biopsy, thereby potentially contributing to early detection and a more favorable prognosis for HNC. Oral neoplasms, particularly squamous cell carcinoma of the head and neck, can be linked to elevated levels of L-Lactate dehydrogenase, measurable in saliva samples, revealing precancerous conditions.
Salivary lactate dehydrogenase is a promising potential biomarker for screening, early detection, and ongoing monitoring of oral potentially malignant disorders (OPMD) or head and neck cancers (HNC), characterized by its simplicity, non-invasive methodology, affordability, and patient acceptance. In order to precisely define the cut-off levels for HNC and OPMD, further research using standardized protocols is recommended.
